He was predeceased by: his parents, Carroll Lee and Della Forrest; his brothers, E. T., Edwin Carroll and James Lee; and his son Steve.
He is survived by: his wife Darlene; his children, Timmy Forrest, Lynne Martin (Rodney), Joyce Ford, Tommy Lewis (Kathy) and Charles Lewis (Lisa); and his grandchildren, Josh Vencill (Kayla), Jeremy Forrest, Laura Hood (Cordell), Erinn Headley (Spencer), Ashley Payne (Cody), Maegan Poad (Billy), Jason Lewis (Leah), Mary Daiger (Brian) and Taylor Lewis. He is also survived by 10 great-grandchildren and many nieces and nephews.
