He was predeceased by: his parents, Herbert M Lewis Sr. and Jesse Hurd Lewis; his brother Thomas H. Lewis; and his daughter Anna Parker. He is survived by: his wife Rita Lewis; his granddaughter Paige Parker; his nephew Harrison Lewis; his extended family Susan Vick Dunn; and also Katie Dunn.
