He was predeceased by: his parents, Mitchell Lee Herbert, Sr. and Carrie Viola Herbert; and his siblings, James Dean Herbert, Mitchell Lee Herbert, Jr., Marie Herbert Mundie and Betty Jo Wilkins.
He is survived by: his caregiver Matt Lugar. He is also survived by nieces, nephews, cousins.
