He was predeceased by: his parents, Frank Richard Forsyth, Sr. and Margaret Katherine Webster Forsyth (Bill); and his brother Donald Ray Forsyth. He is survived by: his wife Nancy Lee Guyton Forsyth; his sons, Frank Richard Forsyth, III and Brett Paul Forsyth; and his grandchildren, Cody Alan Roberts, Kaci Nicole Salgado, Kyle Robert Forsyth and Julie Ann Forsyth. He is also survived by three great grandchildren.

He was an avid contributor to the 12th Man Foundation.
