He is survived by: his brother David Cook; his son Cody Cook (Megan Cook); his grandchildren, Ryatt and Ripp; his son Collin Cook (Cara Cook); his grandchildren, Reid, Cross and Finley; and his former wife Peggy Cook. He is also survived by several nieces and nephews.
