He is survived by: his wife Beverley Dove Cook; his daughter Judy (Steve Andrew) of Amarillo, TX; his grandsons, Eric and Nick; his children, Philip and Chris Cook of Sun City, AZ; his grandchildren, Tory, Christy, Katie and Stephanie; his great grandchildren, Barbara, Tim Hern of Lincoln, MO, Dale, Misty Cook of Allen, TX, Anne, Carl Baldwin of Mansfield, TX, Leah and Danny Parish of Ft. Worth, TX; and also, Sam, Cole, Melissa, Lauren, Kasey, Micah, Aaron, Grant, Megan, Anna, Lynsee, Matthan, Shannon and Titus. He is also survived by six great-grandchildren.
