He was predeceased by: his parents, Don Horn and Barbara Horn. He is survived by: his wife Shannon; his daughter Courtney; his granddaughters, Kylar and Parker; his siblings, Russell Horn and Kelly; his brother-in-law Rich Bartko; and his nieces and nephews, Molly Bembry, Jessica Teeples, Matthew Horn, John Bartko and Evan Horn.
