He was predeceased by: his parents, David Womack and Helen Womack (Roberts); his sister Janet McCoy; and his grandchildren, Tristan Womack and Blake Lewis.
He is survived by: his wife Tammy Lewis of Jacksonville; his children, Zachary Womack (Brittney) of Jacksonville, Rebecca Plyler (Jake) of Arkadelphia and J. T. Lewis (Trisha) of Bullard; and his grandchildren, Emily, Mason, Jackson, Hayden, Abbey, Kennedy, Avery, Madison and John Luke. He is also survived by other nieces, nephews, extended family, and loving friends.
