He was predeceased by: his parents, Theodore Cook and Cora Cook (Lemons); and his siblings, Theodore Jr., Skipper, Louise Lee and Virgie Rowan.

He is survived by: his wife Dianna Kaye Cook (Lamb); his sister Pat Dewitt; his father-in-law Quincy Lamb (Linda); his sisters-in-law, Carla Lamb, Sabra Cook and Sunni Lamb; and his nieces and nephews, Nick and Brylee.
