He was predeceased by: his parents, Monnie Lee Blalock and Ira Beatrice Humphries; and his sister Wanda Jean Mackey. He is survived by: his wife Leveda Stevenson Blalock; his children, David Lee Blalock, Jr. and Tealie Leveda Blalock; his grandchildren, Raven, Michael, Cara, Gabriel and Luke; and his great grandchildren, Ryder, Parker, Miles and Audrey.
