He was predeceased by: his brothers, Norman and Barney; his daughter Natalie Blackburn; and his granddaughter Sherry Blackburn. He is survived by: his wife Maymo Lewis; his son Jay Roger Lewis; his grandchildren, Tammy Blackburn (Rocco Richard), Chris, Chase and Jay Lewis, Jr.; his great grandchildren, Belvin and Colin Richard; and his son-in-law Malcolm Blackburn.
