He is survived by: his wife Kathy Diane Cook; his sons, Brian Cook (Jenny) and Tim Cook (Heather); his mother Thelma Louise; his sister Deborah Thompson; his brother-in-law Larry Kelley; and his friend Donnie Wilson. He is also survived by several nephews, aunts, uncles, and cousins.
