He was predeceased by: his parents, Charles Cook and Eleanor Akers Cook; and his siblings, Ronnie Cook, Carolyn and Melissa Cook.
He is survived by: his siblings, Ricky Cook, Joyce Goodman, Margaret Lay and Joann McKamey; and his stepmother Pauline Cook. He is also survived by Several nieces and nephews.
