He was loved and cherished by many people including: his father Everett Lee Cook; his wife Debbie Cook; his sons, Ashley Cook of Hollow Rock and Lee Cook (Lauren) of Hollow Rock; his mother Ann Cook of Bruceton; his siblings, Cathy Butler (James) of Bruceton and Perry Cook (Alberta) of Camden; and his grandchildren, Tyler Cook (Casi Cook), Sophie Cook and Jaxon Cook.
In lieu of flowers the family request donations be made to St. Jude's Children's Hospital.
