He was predeceased by: his wife Barbara Lee Lewis. He is survived by: his children, LeAnne Cook (Jason), Michael Lewis (Terri) and Mark Lewis; his grandchildren, Burbon, Michael, Zachary, Emily, Jeannie, Samuel, Courtney, Katie, Ava and Kate; and his siblings, Peggy Baker and Bobby Lewis (Anita). He is also survived by 11 great grandchildren.
