He was predeceased by: his father Albert W. Cizek, Jr.. He is survived by: his mother Blanche Svoboda Cizek; his wife Carol Newfield Cizek of York, SC; his daughter Laura Cizek Dushaney (Nelson) of York, SC; his grandsons, Colin Patrick Dushaney and Aaron Richard Dushaney; his brother Albert A. Cizek (Joan) of Higganum, CT; and his nieces, Emma E. Cizek of Denver, CO and Ellen M. Cizek of Boston, MA. He is also survived by several cousins and extended family.
