He was predeceased by: his parents, Theodore S. Hoffman, Sr. and Martha "Molly" Gorman Hoffman; and his sister Kim Ataei-Kachuei. He is survived by: his friend Charlie; his wife Karen Godwin Hoffman; his daughter Whitney H. Whitesides; his son-in-law Quay; his grandchildren, Addison, Evey and Hunter; and his sisters, Lisa H. Rios and Janine H. McMillian. He is also survived by nephews, nieces and his brother-in-laws.
