He was predeceased by: his parents, Robert Asbury Polk and Veda Tuten Polk; and his sister Patricia Wanda Kay Kendall. He is survived by: his children, David B. Polk, Jr. (Melissa) and Tiffany Harbeson; his grandchildren, Trey, Amberly, Andrew, Zoey, Braylon, Ansley, Jace, Makayla, Megan, Sheley, Gatlin and Grayson; and his brother Curtis Polk (Terri).
