He was predeceased by: his parents, Fredrick Simon and Eva Ruth Frasier Koontz; and his brother Gerald Dwain Koontz. He is survived by: his wife Pattie Koontz; his children, Karolyn Koontz Shocknesse, Melissa Koontz Bentley, Detta Gillespie (William) and Jeffery Stone (Misti); and his brothers, Anthony Dale Koontz (Crickett) and Timothy B. Koontz. He is also survived by 8 grandchildren, 8 great grandchildren, and several nieces and nephews.
