He was predeceased by: his parents, Loren W. Forrest and Phyllis Forrest (Harvey); his sister Margaret; his brother-in-law Joe Platt; his brother Glenn; and his sister-in-law Roberta Forrest.
He is survived by: his wife Kathleen Forrest (Sherwood); his children, Amy Nickeson (Jeff Shedden) of Gillett, PA, Brenda Dawson (Pat) of Staunton, VA and Jeff Forrest (Mary) of Sarasota, FL; his grandchildren, Brandon, Jeremy, Matthew, Lauren, Elizabeth, Rebecca and Edward; and his brothers, Loren H. Forrest (Rosemary) of Ulster, PA and Larry Forrest (Connie) of Towanda, PA. He is also survived by several aunts, uncles, nieces, nephews, and cousins.
