He was predeceased by: his parents, Joseph Hasiak and Caroline Hasiak (Nydza); his wife Helen; his son David; his siblings, Joseph Hasiak and Susan Hasiak; and his sister-in-law Patricia Hasiak.

He is survived by: his sister JoAnn Shebby (Bill); his children, Susan Jacobson (Mark, Sr.) of Warrington, Thomas J. Hasiak, Jr. (Sharon) of Warminster and Amy Hasiak of Southampton; and his grandchildren, Shannon Jacobson, Julia Hasiak, Mark Jacobson, Jr. and Joseph Hasiak.

In lieu of flowers, donations can be made to Buxmont Christian Church Pugwugs, 400 Jacksonville Rd. Ivyland, PA 18974.
