He is survived by: his wife Diane L. Unger Middlekauff; his son Brian Andrew Middlekauff; his grandchildren, Zane A. Middlekauff of Shippensburg, Zoe Inez of Chambersburg and Sophia Roan Middlekauff of Chambersburg; his great grandchild Esme Irelia Krieger; and his siblings, Guy Cooper and Barbara Bick. He is also survived by numerous nieces and nephews.
