He is survived by: his wife Teresa L. Kozub (Pavlik); his daughter Terri M. Johns of Destin, FL; his grandchildren, Erin Gless (Edwin) of Anacortes, WA, Craig Johns of Destin, FL and Hayley Bower of Destin, FL; his great grandchild Abigail; and his siblings, Mildred Wright of Saltsburg, Pauline Coleman of Indiana and Daniel I. Kozub of Young Township. He is also survived by nieces and nephews.
