He was predeceased by: his parents, Carl J. Koffke and Lillian R. Koffke. He is survived by: his wife Jacquline D. Koffke; his sons, Derek A. Koffke (Mia) and Jason W. Koffke (Helen); and his granddaughters, Sophia and Chloe.

The family request that instead of flowers, donations go to All Saints' Episcopal Church.
