She was predeceased by: her parents, Frank P. O'Brien and Ann O'Brien; and her siblings, Margaret Conroy and John O'Brien. She is survived by: her husband Donald S. Poliwoda; her children, Donald S. Poliwoda, Jr., Ann Poliwoda, Elizabeth Jacobson (Craig), Sally Poliwoda (Linda), Joseph Poliwoda (Virginia), Mary Anders and Ellen Luby (David); her sisters, Frances Kelly and Katharine Guarini; and her grandchildren, James, Megan, Virginia, Hanna, Alexander, Ellen, Samuel, Brigid and Grace. She is also survived by numerous nieces, nephews, grandnieces and grandnephews.
In lieu of flowers, the family respectfully requests that a donation be made in Pat's name to the St. Francis of Assisi Centennial Renovation Project.
