He was predeceased by: his brother Benjamin. He is survived by: his parents, John C. Fazio, Sr. and Linda E. Fazio; and his siblings, Marla Sprock (Francis), Linda Beyerle (Douglas) and Michael Fazio. He is also survived by numerous aunts, uncles, nieces, nephews, and cousins.
