She was predeceased by: her parents, John E. Smokowitz and Virginia Bradigan (Saxton); her husband Lloyd LaForm; her son Charles "Mickey" Walter; and her sister Joelle Shebelski.

She is survived by: her children, Chris Walter (Katherine) of White Deer, Tracy Boyles (Jim) of Milton and Lisa Edinger of New Columbia; her sisters, Jackie Bartol of Elysburg and Lori Long of Sunbury; and her grandchildren, Tanae, Aaron, Lindsay, Emily, Sonja, Garrett and Ian. She is also survived by three great grandchildren.
