He was predeceased by: his parents, Caldwell "Ted" Forsythe and Lorna Forsythe (Musser); and his granddaughter Keila Waite. He is survived by: his wife Susan Forsythe (Pannebaker); his sons, Ted "T. J." Forsythe of Lewistown and Scott Forsythe (Melissa) of Lewistown; his grandchildren, Brittney Harkness (Taylor), Madyson Forsythe and Landon Forsythe; his great grandchild Liam Harkness; and his sister E. Jayne Young (William) of Williamsport.
