He was predeceased by: his parents, Walter Boltz and Pearl Boltz (Wagner); and his siblings, Edwin, Walter, Leroy Boltz, Florence Rothermel, Mabel Rentschler, Helen Heckman and Grace Phillips.

He is survived by: his wife Judy Ann Boltz (Keller); his children, Bonnie (Larry Werley), Terry Boltz (Dorothy), David Boltz (Donna), Bern Twp and Robert Boltz; his grandchildren, Lisa (Nate), Bethany (Donald), Andrew, Josh (Amanda), Shelly (Scott), Robbie (Jen), Kody (Rachel), Kasy, Shannon (Brad), Bobby (Jessica) and Kirstyn (Kyle); and his siblings, Leon Boltz, Miriam Haas and Arlene Smith.
