She is survived by: her husband Corey Chase; her friend Chyrie; her children, Holley A. Chase and Michael C. Chase; her godmother Mary Curcio; her aunts and uncles, Robert "Bob" Culver, Joe, Jill Curcio, Mike, Jan Curcio, Jim and Becky Curcio; and her cousins, Andy, Nick Culver, Amber, Beth, Chris Curcio, Katie and Julie Curcio. She is also survived by several other nieces and nephews.
In lieu of flowers, memorial contributions may be made to the Northwest Savings Bank account set up by the Bradford YWCA in her children's names, or to a charity of the donor's choice.
