He was predeceased by: his brother Walter Collins. He is survived by: his wife Margaret "Peg" Collins (Morrow); his sons, Robert (Laura) and John Collins (Barb); and his grandchildren, Sean, Jamie, Nick and Megan. He is also survived by 3 nieces.

In lieu of flowers, donations in Jack's memory may be made to charity of donor's choice.
