Kubiak (Castle Shannon, Pennsylvania), born in Chicago, Illinois, who passed away on August 31, 2018, at the age of 69, leaving to mourn family and friends. He was predeceased by: his parent Walter J. of Chicago; his wife Diane E. Kubiak; and his daughter Melanie E. Kubiak. He is survived by: his parent Marion M. Kubiak (Bloedow) of Chicago; his children, Brandon Kubiak (Heather), Mychal Kubiak (Alicia) and Kristyn; and his siblings, John Kubiak (Jayne), Christopher Kubiak and Katherine Kubiak (Jeff Beck).
