He is survived by: his wife Karen Kostyak; his children, Kelly Christie, Michael Kostyak (Heidi) and Michelle Farnam (Matthew); his grandchildren, Alexis, Chelsey, Cayden, Dean, Abel, Colton, Chloe, Ashley and Dylan; his great grandchild Xaine; and his sisters, Christine and Gena. He is also survived by nieces and nephews.
