He was predeceased by: his brother George; and his sister-in-law Norma Long. He is survived by: his wife Janice Ballard Long; his children, Johnny, Jeffrey, Joseph Ballard and Jackie Ballard; his grandchildren, Jakira Ballard [Gabe] Conner, Jeffrey Ballard Jr. and Joshua Ballard; his nephews, Kevin [Sheri] Long and Jerry [Jill] Long; and his niece Linda Long [Jim] Boessenecker.

In lieu of flowers the family asks that contributions be made to the Youth Benefit Golf Tournament (YBGT) at PO Box 107, Mill City OR in honor of Mike or a charity of your choice.
