She was predeceased by: her parents, Charles Dvorak and Marie Dvorak; her husband James Thomas Mootz; her brother Gene; her son-in-law John; and her grandchildren, Skyla and Matthew.
She is survived by: her children, Pat Mootz (Julie Campbell), Melinda Denson, Chuck Mootz (Laurie), Joseph Mootz (Shannon), Margie Lawler (Rick), Matt Mootz (Arlene), Mark Mootz (Stacie), Monica Yoss (Kevin) and Jim Mootz, Jr; and her sister Mildred Stock. She is also survived by children, grandchildren and great-grandchildren.
