He was predeceased by: his son Raymond John Hornak; his parents, John Hlusak and Anne Hlusak; and his sister Irene Hornak. He is survived by: his wife Rosemary Yarchak; his children, Roseanne Hornak and Rick Hornak (Madeline); his grandchildren, Casey Baker, John Hornak and Cate Hornak; and his caregivers, Roberta, Nancy and Paula.
Visitation will be held on Tuesday, November 26th 2024 from 5:00 PM to 7:00 PM at the Fitzgerald Southwood Chapel (3612 E 91st St, Tulsa, OK 74137). A rosary will be held on Tuesday, November 26th 2024 at 7:00 PM at the same location. A funeral Mass will be held on Wednesday, November 27th 2024 at 11:00 AM at the Church of Saint Mary (1347 E 49th Pl, Tulsa, OK 74105).
