He was predeceased by: his mother Cynthia Louise Toler (Fore); and his grandfathers, James B. Curtis and Danny Fore. He is survived by: his father James Walter Curtis; his sons, Jace Curtis of Locust Grove and Jonathan Curtis-Puckett of Locust Grove, Oklahoma; his stepparents, Dawnya Curtis of Locust Grove, Oklahoma and Leon Toler of Locust Grove, Oklahoma; his siblings, James Curtis II (Carrie Ann) of Locust Grove, Donnie Curtis (Ellie) of Waco, Texas, Amber Copeland of Locust Grove and Stormy Toler of Locust Grove, Oklahoma; and his grandmother Evelyn Curtis of Locust Grove, Oklahoma. He is also survived by numerous aunts, uncles, nieces, nephews, cousins, other relatives, and a host of friends.
