He is survived by: his wife Crystal Crumby of Durant, Oklahoma; his son Justin Gonsalves of Milburn, Oklahoma; his nieces and nephews, Shannon Polson of Achille, Gabe Polson of Durant, Oklahoma, Danielle Perry of Achille, Sarah Rose of Achille, Polly Polson of Achille, Mattie Polson of Achille and Erica Polson of Achille, Oklahoma; and his close friends, Billy and Angela Oliver. He is also survived by 3 grandchildren.
