She is survived by: her husband Jim Slade; her children, Jared (Amy Slade) and Jenna (Ronnie Hill); her grandchildren, Hunter, Carson, Taylor Slade, Cadence, Makyah and Josiah Hill; and her siblings, Donald Chambers, Jr. (Merilee), Cindy Erickson (Brian) and John Chambers (Joy).
