He was predeceased by: his parents, Clay Ferst and Avonelle Ferst. He is survived by: his wife Pam Forman; his children, Wyatt and Jesse; his sister Willow Towne of Galion; and his nieces and nephews, Chris and Jaime. He is also survived by other loving family members and friends.
