He is survived by: his siblings, Dr. Terrance Chmielewski of Las Vegas, NV and Mary Jo Samples of Detroit, MI; and his nieces and nephews, Kelly Delvaux of Toledo, Kathleen Rouse of Toledo, Sarah Chmielewski-Sotgiu of New Jersey, Kerry "Bear" Rouse of Toledo, Matthew Chmielewski of Minnesota and Paul Chmielewski of Minnesota.
