Teddy A. Kozlok Obituary
It is with great sadness that we announce the death of Teddy A. Kozlok of Port Clinton, Ohio, who passed away on June 16, 2023, at the age of 74, leaving to mourn family and friends. Family and friends are welcome to leave their condolences on this memorial page and share them with the family.
He was predeceased by: his parents, Andrew Kozlok and Margaret Irene Kozlok (Truex); his brother John; and his sons, Jonathan Andrew and Scott William.
He is survived by: his wife Tanya Diane Eberly. He is also survived by niece and nephew plus many cousins.
