He was predeceased by: his parents, Joseph J. Perlaky and Stephanie Perlaky (Kajfasz) of Toledo, Ohio.. He is survived by: his son Adam Perlaky (Anna); his grandchildren, Arthur, Marit and Charlie Perlaky; and his sister Nikki (Joann Heninger).
