He was predeceased by: his father Jesse Lewis. He is survived by: his wife Cathy Lewis; his children, Shane Lewis, Brad Lewis (Michelle), Chad Lewis and Angie Lewis; and his siblings, Larry Lewis, Patricia Monroe and Deloris Buck. He is also survived by nine grandchildren, seven great-grandchildren and several nieces, nephews, other relatives, and friends.
