He was predeceased by: his parents, Eugene Cook and Rose Mary Cook (Litell); and his close friend Ed Caudill. He is survived by: his wife Jennifer; his children, Samantha Cook and Dustin Blevins; his brother Eugene L. Cook Jr.; his granddaughter Raelyn; and his friends, Jerry Norris, Lisa Norris, Mike Hughes (Margaret), Joe Dranshack (Jennifer), Larry Howell (Sherri) and Dave Crosley (Dee).
