With heavy hearts, we announce the death of Phyllis Jean Fornshell (Miamisburg, Ohio), who passed away on November 4, 2021 at the age of 79. Family and friends are welcome to leave their condolences on this memorial page and share them with the family.

She was loved and cherished by many people including: her parents, Bill Helton and Helen Helton; her husband John; her sons, Todd (Kim) and Bryan (Nicole); her grandchildren, Ashley, Brice (Erika), Brady (Robyn), Brock, Courtney, Brooklyn and Lauren; and her great grandchild Gentry.

In lieu of flowers, donations can be made to the American Lung Association and the American Heart Association in memory of Phyllis Jean Fornshell.
