He is survived by: his wife Hazel Curtis (Applegate); his daughters, Patricia Whitt (Gary) and Sandra Hicks; his grandchildren, Chris Whitt, Amber Hinds and Brandon Curtis; and his great grandchildren, Dillon Pike and Dalton Pike.
