He is survived by: his parents, Brian Formhals and Jennifer Formhals (Evans); his children, Godric and Lilliane; the parent of his children Kandis Grasser (Wooster); his brothers, Alex Formhals (Creston) and Scott Formhals (Fairport Harbor); his grandmother Sharon Formhals (Austintown); his girlfriend Faye Kocher; his uncle Jay Shook; and his cousins, JR Shook and Elijah Formhals. He is also survived by aunts, uncles, and cousins.
