He is survived by: his parents, Monte Cook (Kelly Roberts) of Chesterhill and Nancy Mayle Tippie of McConnelsville; his daughter Haven Cook; his siblings, Danny Mayle of Chandlersville and Angie Cook of McConnelsville; and his nieces and nephews, Myleeigah, Raynna, Madden, Gage, Casey, Dustin and Cody. He is also survived by several aunts, uncles, cousins and friends.
