She is survived by: her husband Mike Crates; her children, Kristopher Crates (Nakoa), Adam Crates (Ginger), David Crates (Stephanie) and Amanda Cummans (Travis); her grandchildren, Jordan Crates, Korbin Crates, Jessica Crates, Landen Crates, Fynnlie Brantley, Cody Crates, Keerstin Collins, Taylor Crates, Adelynn Crates, Aria Crates, Nathan Cummans, Hunter Cummans, Audrey Cummans and Sawyer Cummans; and her brother Dan Shelley (Helen).
In lieu of flowers, memorial contributions may be made to St. Paul United Methodist Church or St. Rita's Hospice.
